The application shall contain:

-request for grant of industrial design;
-data of the applicant;
-the design for which protection is sought and
-description of the design, if necessary for the determination of novelty.

The application shall be submitted and other ingredients and supplements.
The content and form of the other ingredients and attachments to the application are set out in the Regulations for Industrial Design.
